About the Role

We are seeking an experienced and compassionate Assistant Practitioner to join our Child and Adolescent Mental Health Service in Great Yarmouth. As a key member of our team, you will play a vital role in supporting children, young people, and their families in achieving their mental health goals.

Key Responsibilities
  • Assess, plan, and deliver interventions in collaboration with the multidisciplinary team.
  • Work autonomously within clearly defined boundaries to meet the needs of service users.
  • Develop and maintain a competence portfolio, adhering to the Competency Framework.
  • Participate in ongoing assessments of service users and contribute to multidisciplinary team meetings.
  • Deliver person-centered care, promoting the principles of Recovery and safe practice.
  • Liaise with external organizations to ensure holistic care and promote partnership working.
  • Hold and manage an identified caseload, ensuring accurate reflection of service users' needs in health record documentation.
  • Provide therapeutic interventions, including 1:1 sessions and group activities, and demonstrate an understanding of pharmacology.
  • Carry out basic physical health checks and respond to any concerns, ensuring service users' physical health needs are met.
Requirements
  • Care certificate or relevant equivalent.
  • Foundation Degree in Health and Social Care/Mental Health or apprenticeship in H&SC/Mental Health L5.
  • Working experience with service users with mental health needs, within a multidisciplinary team, and knowledge of the CPA process.
  • Ability to communicate widely in a clear manner, problem-solve, and demonstrate a confident and approachable demeanor.
  • Good time management and ability to meet deadlines, with a flexible attitude and willingness to travel independently.
