About the Role

We are seeking a highly skilled and dedicated Assistant Practitioner to join our Child and Adolescent Mental Health Service in Great Yarmouth. As a valued member of our team, you will play a crucial role in supporting children, young people, and their families in achieving their mental health goals.

Key Responsibilities
  • Assess, plan, and deliver interventions in collaboration with the multidisciplinary team.
  • Engage with service users and their carers to understand their needs and develop personalized care plans.
  • Work independently and as part of a team to deliver high-quality care and services.
  • Participate in ongoing assessments and reviews of service users' needs, ensuring accurate and up-to-date documentation.
  • Develop and maintain relationships with internal and external agencies to ensure seamless care coordination.
  • Communicate complex information to a wide range of professionals and service users in a clear and sensitive manner.
  • Provide updates and reports on service users' needs and care, both written and verbal.
Requirements
  • Foundation Degree in Health and Social Care or Mental Health, or equivalent qualification.
  • Experience working with service users with mental health needs, preferably in a multidisciplinary team setting.
  • Working knowledge and understanding of the Care Programme Approach (CPA) process.
  • Excellent communication and problem-solving skills, with a confident and approachable manner.
  • Ability to work independently and as part of a team, with good time management and organizational skills.
  • Understanding of mental health issues, risk assessment, and confidentiality.
