**Job Title:** Community Care Assistant
**Estimated Salary:** £20,000 - £23,000 per annum
We are seeking a caring and compassionate individual to join our team as a Community Care Assistant. In this role, you will work closely with patients and families to provide high-quality care and support in their own homes.
The successful candidate will have a minimum of 2 years' experience as a Healthcare Assistant/Worker in a health or social care setting and hold a valid UK driving licence. They will also have excellent communication and interpersonal skills, with the ability to work under pressure and prioritize tasks effectively.
In this role, you will be responsible for providing care and support to patients with a range of needs, including those with physical disabilities, terminal illnesses, and elderly individuals. You will work collaboratively with other healthcare professionals to develop and implement care plans that meet the unique needs of each patient.

**Key Responsibilities:**- Provide high-quality care and support to patients in their own homes

- Relevant level 3 qualification (NVQ/QCF Diploma or certificate of care) or equivalent
