Social Worker for Vulnerable Adults

Randstad Care is seeking a skilled Social Worker to join our team, specializing in supporting vulnerable adults with learning disabilities. As a key member of our multidisciplinary team, you will work closely with individuals, their families, and other healthcare professionals to deliver coordinated care and support.
About the Role:
In this role, you will be responsible for conducting thorough assessments to understand individual needs, developing personalized care plans, and collaborating with various services to ensure timely access to support and resources. You will also be required to conduct mental capacity assessments, make DoLS applications, and provide immediate support and intervention in response to crises or emergencies.
Requirements:
To be successful in this role, you will need a relevant social work qualification and at least 2 years of experience in adult social work, including assessment and care planning. You should have excellent communication and interpersonal skills, as well as the ability to work collaboratively as part of a multidisciplinary team.
