About the Role

We are seeking a dedicated social worker to join our Adults with Learning Disabilities and Autism Team, working with adults aged 18 and above who have been diagnosed with a Learning Disability and/or Autism in Telford and Wrekin.

This is an exciting opportunity for a social worker to join a passionate team that supports individuals to live fulfilling lives as valued members of their community. We work in a person-centred way to enable adults to reach their full potential, maximize their social inclusion, and achieve positive outcomes.

You will be expected to work closely with individuals, their carers, and families to build a Support Plan, incorporating a range of interventions that provide holistic, evidence-based, and community-delivered provisions.

Key Responsibilities
  • Work in a person-centred way to support individuals with Autism and/or Learning Disabilities
  • Develop and implement Support Plans in collaboration with individuals, carers, and families
  • Provide holistic, evidence-based, and community-delivered interventions
