Main Responsibilities:- Assess, diagnose, and manage patients with complex medical conditions, providing expert care and treatment.
- Develop and implement individualized treatment plans, working closely with patients and their families to achieve optimal health outcomes.
- Prescribe and review medications, ensuring safe and effective use.
- Work collaboratively with our multidisciplinary team to provide comprehensive patient care, including referrals to secondary or specialist care as needed.
- Contribute to the development and implementation of health promotion and wellbeing programs, supporting the health and wellbeing of our patients.

Person Specification:- Postgraduate degree qualification in Clinical Practice or equivalent.
- Registered Healthcare Practitioner.

- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
