Job Title: Care Management Specialist

About the Role:
As a Care Management Specialist with Voyage Care Ltd, you will play a critical role in leading our Small Six bed residential service in Bedford. You will be responsible for managing a team of colleagues to deliver high-quality care and services that meet individual needs and goals. This is an exciting opportunity for an experienced care professional who is passionate about enhancing the quality of life of individuals with Learning disabilities, mental health needs, and challenging behaviours.

You will have a strong background in care management, with a proven track record of delivering high-quality care and services that meet individual needs and goals. You will possess excellent leadership skills, with the ability to motivate and develop a team to achieve their full potential. Your passion for person-centred care and your commitment to delivering high-quality services that meet individual needs and goals are essential.

The Job:
Your primary responsibility will be to lead a team of colleagues to deliver high-quality care and services that meet individual needs and goals. This will involve developing and implementing person-centred support plans, conducting care needs assessments, recruiting and inducting new colleagues, completing supervisions and appraisals, and attending multi-disciplinary meetings. You will also take part in the on-call rota, ensuring that our service operates 24/7.

Key Responsibilities:
* Lead a team of colleagues to deliver high-quality care and services that meet individual needs and goals.
* Develop and implement person-centred support plans to enhance the quality of life of individuals supported.
* Conduct care needs assessments to identify and address individual needs.
* Recruit and induct new colleagues, ensuring they receive the necessary training and support to perform their roles effectively.
* Complete supervisions and appraisals to ensure team members are meeting performance expectations.
* Attend multi-disciplinary meetings to contribute to the development of individual support plans and ensure effective communication between teams.
