Job Description

The Hywel Dda University Health Board is seeking a qualified and experienced Adult Community Practitioner to join their team in the North Ceredigion area.

This role requires a professional with significant experience working in an acute adult mental health setting, possessing a sound knowledge of current issues pertaining to mental health, crisis intervention, and home treatment.

The successful candidate will be responsible for care planning, assessment of individuals in distress, involvement in clinics, and collaborating with other teams to provide comprehensive care outcomes.

The ability to travel between sites efficiently is essential for this position.

Key Responsibilities:

  • Care planning and management for adults with mental health difficulties
  • Assessment and risk management for individuals in crisis
  • Collaboration with multidisciplinary teams for comprehensive care
  • Travel between sites as required

Requirements:

  • Professional qualification or registration (e.g., RMN, Occupational Therapist, Social Worker)
  • Post-basic education to postgraduate diploma level
  • Post-registration experience

Estimated Salary: £35,000 - £45,000 per annum
