**Job Overview:**

Hoop Social Work is recruiting a Social Worker to join their Triage and Response Team in Bristol. The successful candidate will work closely with healthcare colleagues to provide timely support to service users, ensuring seamless patient care transitions.

The ideal candidate will have a strong background in social work, excellent communication skills, and a passion for enabling service users to leave the hospital in a timely manner.

Responsibilities:

  • Completing Care Act assessments and mental capacity assessments
  • Developing comprehensive discharge plans and collaborating with medical staff

Working Arrangements:

  • Full-time hours required
  • Requirement to be in the office daily (unless reasonable adjustments apply)
