Job Title: Nurse Assessor
Job Summary:
Gray Healthcare is seeking an experienced Nurse Assessor to lead on clinical assessments and ensure the delivery of individualized support packages. As a key member of our team, you will work closely with the Assessment and Contracts Manager to ensure packages are deliverable and in line with our model of support.
Main Responsibilities:
- Completing desk-based and face-to-face assessments of referred individuals for support.
- Ensuring an APIE approach - Assessing, planning, implementing, and evaluating.
- Reviewing reports and evidence outcome measures to inform clinical practice, business performance, and quality measures.
- Able to identify and mitigate complex risks for a community setting.
- Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
- Identifying measures to ensure individuals can be enabled through positive support.
- Attending internal and external MDT meetings to further inform assessment and risk mitigation.
- Building professional working relationships.
- Effective time management.
- Record keeping, ensuring accurate, safe, and contemporaneous records are maintained.
Requirements:
- Registered Mental Health Nurse or Occupational Therapist.
- Experience of assessing and working with individuals with complex needs, including mental health, learning disability, ABI, personality disorder, autism, and forensic backgrounds.
- Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community.
- Applied knowledge of the Mental Health Act, CTOs, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making.
- Expertise in assessment and management of risk, with strong report writing skills and computer literacy.
- National travel and overnight hotel stays may be required.
- Full UK Driving Licence and access to a vehicle.
