Job Summary:

Gray Healthcare is seeking a highly skilled and experienced Clinical Lead Nurse to join our team. As a Clinical Lead Nurse, you will be responsible for leading clinical assessments and ensuring that we deliver individualized support packages to our clients.

Main Responsibilities:

  • Completing desk-based and face-to-face assessments of referred individuals for support.
  • Ensuring an APIE approach - Assessing, planning, implementing, and evaluating.
  • Reviewing reports and evidence outcome measures to inform clinical practice, business performance, and quality measures.
  • Able to identify and mitigate complex risks for a community setting.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Identifying measures to ensure people can be enabled through positive support.
  • Attending internal and external MDT meetings to further inform assessment and risk mitigation.
  • Building professional working relationships.
  • Effective time management.
  • Record keeping, ensuring that accurate, safe, and contemporaneous records are maintained.

Requirements:

  • Aegistered Mental Health Nurse, Occupational Therapist - ESSENTIAL.
  • Experience of assessing and working with people with a variety of complex needs around their mental health, learning disability, ABI, personality disorder, autism, and forensic backgrounds.
  • Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community and a collaborative, solutions-focussed approach to any issues associated with this.
  • Applied knowledge of the Mental Health Act, CTO's, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making.
  • An expert in assessment and management of risk, you will be an innovative thinker and natural communicator with strong report writing skills and computer literacy and an ability to work under pressure.
  • National travel and overnight hotel stays may be required.
  • Full UK Driving Licence and access to a vehicle.
