Enablement Specialist

Job Summary
The Nairn & Arderseir Reablement team provides a vital service focusing on assessments at home and swift discharges to residents of the area. As an Enablement Worker, you will assist with assessing and utilising an enablement approach, identifying outcome plans for individuals and their families/carers.
Key Responsibilities
- Maximise independence and support individuals to remain in their own home
- Facilitate prompt discharge from hospital and support anticipatory care planning to prevent readmission or admission
- Work with the Scottish Social Service Council to ensure registration within 3 months of appointment
