Enablement Worker

Job Summary: We are seeking an experienced Enablement Worker to join our Nairn and Ardersier Reablement Care at Home Team. The successful candidate will be responsible for assessing and utilizing an enablement approach to identify outcome plans for individuals and their families/carers.
Key Responsibilities:
- Assess and develop enablement plans to maximize independence and support individuals to remain in their own homes.
- Collaborate with healthcare professionals to facilitate prompt discharge from hospital and support anticipatory care planning to prevent readmission or admission to a hospital.
- Work closely with individuals, families, and carers to understand their needs and develop personalized support plans.
Requirements:
- Registration with the Scottish Social Service Council within 3 months of appointment.
- Ability to work effectively in a busy reablement and care at home environment.
- Strong communication and interpersonal skills.
