Care Assistant

At Live Connected, we're passionate about making a difference in people's lives. As a Support Worker, you'll play a vital role in transforming the lives of adults with mental ill health, learning disabilities, dementia, and complex needs.
Our Supported Living services provide quality care and support to help individuals live independently and achieve their goals. As a Support Worker, you'll work closely with our team to deliver personalized care and support, ensuring each individual receives the best possible outcomes.
Key Responsibilities:
- Undertake daily living tasks as agreed with service users, their families, and professionals involved in their care plan.
- Work within our policies and procedures to ensure high-quality care and support.
- Complete Health and Safety checks and ensure safe medication delivery and ordering.
- Provide opportunities for service users to express their preferences and maintain their independence.
- Manage service users' behaviors in the least restrictive way possible.
- Ensure accurate and efficient handovers and documentation.
- Work collaboratively with our Multi-Disciplinary Team to support service users' emotional and physical well-being.
Requirements:
- 6 months' experience in a Supported Living service.
