Job Summary

We are seeking a highly motivated and experienced Senior Health Care Support Worker to join our Community Learning Disability Health Team in Barnsley. This is a full-time Band 3 permanent post.

Main Duties

The post holder will work under the supervision of registered health care professionals to deliver planned programmes of care and treatment to designated service users. Key responsibilities include:

  • Providing high-quality care and support to individuals with learning disabilities
  • Developing and maintaining meaningful therapeutic relationships with service users
  • Assessing and addressing the physical and emotional needs of service users
  • Working collaboratively with multidisciplinary teams to achieve positive outcomes for service users
  • Participating in the management and support of individuals who exhibit challenging behavior
Key Result Areas

The post holder will be responsible for:

  • Delivering excellent levels of customer service to people
  • Working with a degree of independence within defined parameters
  • Contributing to the initial assessment, admission, and discharge process
  • Acting as a link or associate link for a specific area of practice
  • Clearly and accurately contributing to care documents and reports
Person Specification

The ideal candidate will possess:

  • GCSE A-D or grade 3-9 in Maths and English or equivalent
  • Apprenticeship level 3 or equivalent in Health & Social Care
  • Experience of working in a caring environment at a Band 2 or equivalent level of competence
  • Excellent communication and interpersonal skills
  • Ability to work effectively and professionally as part of a multidisciplinary team
Training and Development

The post holder will be required to undertake all appropriate Trust mandatory training requirements and subsequent refresher training relevant to the area.
