About the Role

We are seeking a skilled Clinical Healthcare Specialist to join our team at NHS North Central London Integrated Care Board. This is an exciting opportunity for a dedicated professional to contribute to the implementation and delivery of the National Framework for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).

The successful candidate will have expertise in assessing individuals with mental health needs and/or dementia, completing timely CHC and FNC reviews, and providing specialist nursing advice and support.

Key Responsibilities
  • Liaising with families, providers, and local authority partners using exceptional interpersonal and communication skills.
  • Undertaking End of Life care reviews, following national guidance for individuals Fast Tracked through the CHC system.
  • Exercising professional judgement when communicating information on behalf of the CHC service.
  • Providing specialist clinical advice to secondary care, primary care, nursing homes, domiciliary care providers to facilitate the CHC process.

The ideal candidate will possess diploma-level specialist training, experience, and short courses or equivalent level of experience. They should also demonstrate evidence of continuing professional development, experience working in a healthcare environment, and supervision and teaching of others.

Requirements
  • A strong understanding of the National Framework for Continuing Healthcare and its application in a community setting.
  • Ability to communicate effectively across the health, social, and independent care community.
  • Supervisory experience and ability to lead education programs.

The salary range for this position is £44,806 to £53,134 per annum, including HCAS. The role requires a Disclosure and Barring Service (DBS) check due to the nature of the work.
