About the Role
The Midlands Partnership NHS Foundation Trust is seeking a highly skilled Pharmacy Technician to join our team as a Medicines Optimisation Specialist. This is an exciting opportunity to lead on the development and delivery of an Integrated Home First Medicines Optimisation Service, improving safe transfer and use of medicines across clinical interfaces.
Key Responsibilities
- Provide expert advice and support on all aspects of medicines optimisation to ensure use of medicines is optimised and meets safety, statutory and governance requirements.
- Work independently to provide a seamless pathway for medication support from in-patient to community services/patient.
- Develop and implement prioritisation matrices, referral pathways, and structured medication reviews to reduce the risk of harm from taking medicines.
- Support reduction in medication escalation to on-call clinical teams and ensure that requests are processed safely and prioritised to match clinical urgency.
- Provide professional advice and support to other health and social care professionals on the safe prescribing, handling, and administration of medicines.
- Identify, develop, and provide support to meet medicines-related training needs for clinical and non-clinical staff.
- Develop partnerships and facilitate collaborative working between multidisciplinary teams to optimise medicine-related discharge processes and ensure safe transitions of care.
Person Specification
- BTEC and NVQ3 (or equivalent) in Pharmaceutical Science.
- Registered as a Pharmacy Technician with the General Pharmaceutical Council.
- Nationally recognised qualification as accredited Medicines Management Technician (desirable).
- Nationally recognised Accredited Checking Technician Certificate (desirable).
