Job Summary:

The Stockport NHS Foundation Trust is seeking a highly skilled and dedicated individual to fill the role of District Nurse. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care in patients' homes, residential homes, and treatment rooms. This position requires excellent communication and interpersonal skills, with the ability to work independently and as part of a team.

Key Responsibilities:

- Provide high-quality patient care in patients' homes, residential homes, and treatment rooms
- Work closely with other healthcare professionals to ensure seamless care coordination
- Develop and maintain strong relationships with patients, families, and caregivers
- Stay up-to-date with current clinical issues and best practices in out-of-hospital care

Requirements:

- Current registration with the NMC
- Grade C (or equivalent) in GCSE English
- Diploma in Nursing Practice or degree (desirable)
- Evidence of teaching and assessing students and junior staff members (essential)
- Effective time management and ability to work under pressure
