Job Summary

The Elms Surgery is seeking a skilled and dedicated General Practitioner to join our team for 4-6 sessions per week. As a GP, you will be responsible for providing high-quality patient care, undertaking a variety of practice duties, and working collaboratively with our team to deliver excellent healthcare services.

Main Responsibilities

  • Undertake face-to-face consultations, telephone consultations, and home/care home visiting to provide patient care.
  • Action bloods, tasks, and prescriptions, and deal with queries as part of the day-to-day schedule.
  • Make professional decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers.
  • Assess patient health needs and develop care plans for patients with long-term conditions.
  • Provide counselling and health education to patients.
  • Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
  • Prescribe in accordance with the practice prescribing formulary and work to ICB guidelines.
  • Support and supervise physician associates to ensure smooth practice running.
  • Manage and support ongoing palliative care and elderly care case loads.

Qualifications

  • Recently qualified or experienced GP.
  • Annual appraisal and revalidation (when appropriate).
  • On the NHSE medical performers list.

Experience

  • Desirable: Specialist interest in elderly or palliative care, long-term condition management, or teaching.
