About the Role

Southern Health NHS Foundation Trust is seeking a dedicated and enthusiastic Mental Health Practitioner to join their team. As a key member of the multi-disciplinary team, you will be responsible for supporting the assessment and care planning for service users with severe and enduring mental illness.

Main Responsibilities

  • Act as care co-ordinator to an agreed caseload of service users
  • Undertake ongoing assessment, treatment and evaluation for service users known to the service
  • Support the delivery of therapeutic interventions either individually or within a group setting
  • Support the transformation of the team to deliver local evidence-based care

Person Specification

  • Registered Nurse, Social Worker, Mental Health Practitioner or equivalent qualification
  • Significant post-qualifying experience
  • Mentorship course
  • Clinical supervision training
