About the Role

We are seeking an experienced Advanced Practice Physiotherapist to join our team at Connect Health Group Limited. As a key member of our rheumatology service, you will be responsible for managing a complex clinical caseload and working closely with experienced rheumatology consultants.

Key Responsibilities

  • Manage a varied and complex clinical caseload, utilizing advanced practice expertise to deliver high-quality patient care.
  • Collaborate with experienced rheumatology consultants, benefiting from their expertise, mentorship, and insights regarding the latest evidence-based treatments.
  • Shape the future of rheumatology healthcare within our community service setting, driving innovation and improvement.
  • Deliver one-stop clinics, working directly with consultants to streamline patient care and provide individualized treatment plans.
  • Triage referrals from GPs, directing patients effectively and efficiently.
  • Lead and participate in team meetings for holistic patient care, providing specialty advice across the pathway.
  • Engage in clinical research, advancing medical knowledge and contributing to the development of new treatments and therapies.
  • Deliver educational sessions to raise awareness of rheumatological conditions, promoting health education and patient empowerment.
