About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Andover Health Centre Medical Practice. As a key member of our Primary Care Network, you will play a vital role in supporting patients with complex social needs and promoting their overall health and wellbeing.

Key Responsibilities
  • Community Outreach and Engagement
    • Develop and maintain strong relationships with local community groups, voluntary sector organisations, and statutory agencies to promote social prescribing and encourage referrals.
    • Work collaboratively with these partners to identify unmet needs and gaps in community provision, and develop new groups and services where necessary.
  • Personalised Support and Signposting
    • Provide one-to-one support to patients, empowering them to take control of their health and wellbeing through non-medical interventions.
    • Signpost patients to community groups, activities, and statutory services that meet their needs, and ensure they are comfortable and included.
  • Data Capture and Evaluation
    • Work sensitively with patients, their families, and carers to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing.
    • Encourage feedback and share stories about the impact of social prescribing on patients' lives.
  • Education and Training
    • Undertake training for Social Prescribing Link Workers as set out by the Personalised Care Institute.
    • Maintain your own continuing professional development, keeping up to date with developments around Mental Health and Wellbeing.
Requirements
  • Personal Qualities and Attributes
    • Ability to listen, empathise with people, and provide person-centred support in a non-judgemental way.
    • Able to get along with people from all backgrounds and communities, respecting lifestyles and diversities.
    • Commitment to reducing health inequalities and proactively working to reach people from all communities.
  • Experience and Qualifications
    • Experience of working directly in a community development context, adult health, and social care, learning support, or public health/health improvement.
    • Experience of supporting patients with healthy weight management.
    • Experience of supporting people, their families, and carers in a related role.
  • Knowledge and Skills
    • Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities.
    • Knowledge of IT systems, including ability to use word processing skills, emails, and the internet to create simple plans and reports.
