We are seeking a dedicated and compassionate Young Person's Health Advocate to join our team at West Hertfordshire Teaching Hospitals NHS Trust. As a key member of the Paediatric Service, you will play a crucial role in advancing the care of patients transitioning from paediatric to adult services.

This is an excellent opportunity for a senior nurse to take a leading role in shaping and developing the provision of care for this patient group. You will collaborate closely with the Paediatric Team, Paediatric Consultants, Adult Medicine Team, MDT members, and both clinical and administrative colleagues across the Paediatric and Adult services.

The successful candidate will have experience of working with young people who have long-term medical conditions, managing patients with multiple complex health conditions, and successfully leading and implementing service improvement projects. They will also have a good understanding of the health and social care environment and roles and responsibilities within it.

In return, we offer a competitive salary of £48,513 per annum, plus benefits including pension scheme, annual leave, and training opportunities. The post holder will work 37.5 hours per week (Monday to Friday).

Main Responsibilities:

  • Ensure that the transition of care from children's to adult services is appropriately managed and delivered effectively within West Herts.
  • Be a key member of the Children and Young People's team while supporting effective communication and stakeholder management.
  • Ensure that the views of young people are intrinsic in the models of care developed.
  • Support staff to access relevant data and translate service data into meaningful performance information.
