We are seeking an experienced Children's Nurse to join our team as a Care Coordinator for children with complex needs. The successful candidate will work closely with families, schools, and healthcare professionals to provide high-quality care and support.

Job Summary

\

The main purpose of this role is to coordinate and deliver specialist nursing services for children and young people with complex health needs attending special schools in West Hertfordshire. You will work within the Special School Nursing service to provide year-round support, working in partnership with children, young people, and their families, together with other professionals and agencies to meet health, development, and safeguarding needs.

Main Duties

  2. Deliver a specialist nursing service for children and young people with complex health needs attending special schools.\
  3. Work within the Special School Nursing service to provide year-round support and coordination of communication amongst multi-agency professionals.\
  4. Act as a resource for professionals, championing the needs of children and young people with disabilities and build effective working relationships across organisational boundaries.\
  5. Ensure the voice of the child is heard, acting as an advocate for the children, young people, and their families.\
  6. Support children and young people to return to school following episodes of prolonged illness through attendance at discharge planning meetings, multi-disciplinary team meetings, and where appropriate, home visits to assess potential changes in health needs which will impact on their ability to attend.\

Requirements

  • \
  • Registered Children or Learning Disabilities Nurse.\
  • First Degree in a relevant subject or equivalent qualification and experience.\
  • Evidence of Continuing Professional Development.\
  • Sound knowledge of safeguarding issues and policy/legal framework.\
  • Knowledge and skills in the management of long-term conditions and complex needs.\
\

Working Arrangements

\

This is a full-time role, working 37.5 hours per week. The salary for this position is £40,000 - £55,000 per annum, depending on experience. A car driver with a full valid UK driving licence is essential for this role, as you will be required to travel between schools and other locations.
