We provide health and social care services to people with mental ill-health, physical ill-health, and learning disabilities across Hertfordshire, Buckinghamshire, Essex, and Norfolk.

We are looking for a skilled Social Worker to join our team, providing high-quality services to adults experiencing mental ill-health. This role involves working as part of a multi-disciplinary team to deliver assessment, care planning, and coordination of care from a social perspective.

The successful candidate will have experience of undertaking assessments, focused on personalisation, choice, and control, and care planning. They will also have knowledge and understanding of personal budgets and direct payments, as well as experience of multidisciplinary working and partnership working with service users and carers.

We offer excellent communication skills and ability to analyse service user and carer information. The ideal candidate will be able to undertake small-scale audits and surveys, and have a strong understanding of legislation and statutory guidance.
