About the RoleWe are seeking a skilled Complex Needs Practitioner (Family Practitioner) to join our team in Aylesbury. As a key member of our multi-disciplinary team, you will work with individuals and families affected by substance use, providing person-centred interventions and support to empower them to reduce risk, increase resilience, and achieve positive outcomes.
Key Responsibilities- Engage with individuals and families affected by substance use, using a person-centred approach to empower them to reduce risk and increase resilience.
- Provide psychosocial interventions and support to individuals and families, working collaboratively with our partners and stakeholders.
- Work flexibly across the community and within our multi-agency partnership settings, including health and social care.
- Develop and maintain effective relationships with clients, colleagues, and partners, ensuring a high level of customer satisfaction and service delivery.
- Contribute to the development and implementation of effective interventions and support plans, ensuring they are tailored to the needs of individuals and families.
To be successful in this role, you will need:
- Experience of working with individuals and families affected by substance use, preferably in a family support or substance misuse setting.
- Knowledge and understanding of family interventions, substance misuse, and related issues.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, colleagues, and partners.
- A flexible approach, with the ability to work across the community and within our multi-agency partnership settings.
- Excellent record-keeping and report-writing skills, with the ability to maintain accurate and up-to-date records.
