The Manchester Thrive Hub is a pioneering initiative that brings together health, care, education, and social services to provide integrated support for children and young people. As a Thrive Hub Navigator, you will play a critical role in ensuring that these services are delivered effectively, enabling young people to thrive in their local communities.
With a strong focus on community outreach and partnership working, you will work closely with local stakeholders to develop close relationships with the range of services available in the area. Your key aim will be to ensure that children, young people, and carers are able to access the support they need, when they need it.
Main Duties- Act as an enabler and systems supporter, working with local services to deliver integrated community responses to meet the wellbeing needs of children, young people, families, and carers.
- Mapped and developed close working relationships with local services, ensuring that all stakeholders, including children, young people, and carers, are supported.
- Signed post and supported individuals into services in the community, predominantly VCSE sector.
As a Thrive Hub Navigator, you will have a deep understanding of the complex needs of children and young people, as well as the importance of building strong relationships with local stakeholders. You will be an excellent communicator, able to work collaboratively with a range of professionals to achieve positive outcomes.
