The Role

This exciting opportunity is available for an experienced Mental Health Practitioner to join our Memory Assessment Service East (CMAS) on a secondment or fixed-term basis, covering maternity leave. As a Dementia Practitioner Transformation Specialist, you will play a vital role in our multidisciplinary team, working closely with nurses, occupational therapists, support workers, consultants, and clinical psychologists to deliver high-quality care to patients with dementia.

Your primary responsibilities will include:

  • Completing comprehensive assessments to investigate cognitive changes and symptoms of dementia
  • Delivering diagnoses of dementia to patients and their families in a sensitive and supportive manner
  • Participating in regular team teaching and development opportunities to enhance your skills and knowledge
  • Attending monthly management and clinical supervision sessions to ensure effective collaboration and continuous improvement
Requirements

To succeed in this role, you must have:

  • A strong understanding of dementia subtypes and their impact on patients and their families
  • An ability to travel independently to various locations across Suffolk
  • Excellent communication and interpersonal skills, enabling you to build strong relationships with patients, families, and healthcare professionals

We offer a competitive salary package, including:

$45,667 - $61,129 per annum, depending on experience

27 days annual leave increasing to 33 days after three years of service, plus bank holidays
