We are seeking a dedicated and passionate individual to join our team at the Stroke Association as a Community Connect Coordinator in Carmarthenshire. This exciting role will enable you to make a meaningful difference in the lives of stroke survivors and their families by providing innovative support solutions and empowering them to live independent lives in the community.

The Community Connect Coordinator will:

  • Provide personalized information, advice, and support to new stroke survivors and their carers.
  • Work collaboratively with Social Care, NHS, and 3rd sector colleagues to identify and address the needs of stroke survivors and carers.
  • Support stroke survivors to make informed lifestyle changes that will help them prevent further strokes.
  • Manage a team of volunteers to deliver the Community Connect service.
  • Promote stroke-friendly communities and connect people to their community and peers.

This role requires the post holder to travel across the locality to connect people to their community and peers. Candidates must be able to demonstrate how they can meet this requirement of the role.

To be successful in this role, you must be a resident in the UK and have the right to work in the UK.
