Job Title: Mental Health Practitioner - Community Setting

We are seeking a skilled Mental Health Practitioner to join our team in a community setting. The successful candidate will work closely with GPs and other healthcare professionals to provide high-quality care and advice to patients with mental health problems.

The ideal candidate will have significant post-registration experience of working with patients within a Community Mental Health setting, with a strong understanding of mental health issues and public health agendas. They will also possess excellent clinical supervision skills and be able to work effectively in a multi-agency community setting.

The role will involve assessing and managing clinical risk, working in a primary care setting, and liaising closely with Secondary Care Mental Health Services, Local Primary Mental Health Support Services, and Third Sector Organisations to ensure cohesive pathways of care.

Main Responsibilities:
  • Provide high-quality care and advice to patients with mental health problems
  • Work as part of a multidisciplinary Community Health and Wellbeing Team within a community setting to support GPs and other practice staff
  • Liaise closely with Third sector organisations, LPMHSS, and Secondary Care Mental Health Services including crisis teams
