About the Role

We are seeking an experienced Primary Care Mental Health Practitioner to join our Community Health and Wellbeing Team. The successful candidate will work closely with GPs and other practice staff to provide specialist mental health support and promote positive mental health behaviors.

Main Responsibilities
  • Provide high-quality care and advice to patients with mental health problems, including assessment and signposting to appropriate community services.
  • Work as part of a multidisciplinary team to support GPs and other practice staff in providing community-based mental health services.
  • Liaise closely with secondary care mental health services, local primary mental health support services, and third sector organizations to ensure seamless pathways of care.
Requirements
  • Professional registration as a Registered Mental Health Nurse (RMN) or equivalent.
  • Degree in a mental health-related subject or equivalent experience.
  • Experience of working in a community mental health setting, including assessment and management of clinical risk.
  • Ability to work independently and as part of a team.
