Main Responsibilities:- Develop and implement health promotion and well-being programmes to improve patient outcomes.
- Manage patients with acute and chronic medical conditions, providing expert advice and care.
- Implement and evaluate individualised treatment plans for patients with chronic diseases.
- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide advanced nursing care to patients, adhering to clinical guidelines and evidence-based practice.
- Provide wound care, including ulcer and Doppler assessments.
- Coordinate specialist clinics, such as ear syringing and contraception.
- Collect and process pathological specimens, requesting pathology services as necessary.
- Interpret pathology and test results, referring patients to secondary or specialist care when required.
- Maintain accurate clinical records, ensuring compliance with legislation and best practice.
- Develop and implement protocols for chronic disease management, well woman and well man clinics, and travel clinics.
- Lead the development of an effective call/recall system and immunisation programme.
- Chaperone patients during examinations and procedures.
- Prioritise health issues and intervene appropriately to prevent adverse outcomes.
- Support the team in managing clinical emergencies and safeguarding vulnerable patients.
- Recognise and refer patients presenting with mental health needs.
- Implement vaccination programmes for adults and children, adhering to guidelines and best practice.
- Prescribe medications, adhering to guidelines and best practice.
- Support patients in using their prescribed medications, reviewing annually as required.
- Contribute to practice targets, complying with local and regional guidance.
- Liaise with external services and agencies to ensure patient support.
- Delegate clinical responsibilities, ensuring safe practice and adherence to scope of practice.
- Support the clinical team in safeguarding matters, adhering to local and national policies.
- Understand and adhere to practice and local policies for substance abuse and addictive behaviour.
- Registered Nurse with postgraduate diploma or degree in Advanced Practice.
- Qualified Nurse Prescriber, Qualified Triage Nurse, Minor Illness Qualification, Teaching Qualification, ALS, and PALS.
- Experience working in primary care, chronic disease management, prescribing, and medication reviews.
- Experience working as a practice nurse or community nurse.
- Wound care, removal of sutures and staples, ECGs, venepuncture, new patient medicals, chaperone procedure, requesting pathology tests, and processing results.
- Travel medicine, diabetes, hypertension, asthma, spirometry, CHD, immunisations, women's health, and evidence-based practice.
- Broad knowledge of clinical governance, accurate clinical note-taking, and working within scope of practice.
