About the Role

We are seeking an enthusiastic Occupational Therapist to join our Sutton Health and Care at Home service. As part of our multidisciplinary team, you will play a vital role in preventing hospital admissions, accelerating discharge from emergency departments, and providing supported discharge through intermediate care.

Key Responsibilities
  • Work collaboratively with the MDT team to deliver high-quality care to patients and families.
  • Contribute to the development and implementation of innovative care pathways.
  • Provide clinical teaching and supervision to support workers.
  • Participate in research and quality improvement initiatives.

Requirements
  • Degree in Occupational Therapy or equivalent qualification.
  • HCPC registration and relevant postgraduate qualifications.
  • Significant post-registration experience in a community setting.
  • Experience of working with support workers and conducting training/teaching sessions.
