Job Summary

Piers Meadows Recruitment is seeking a skilled Band 6 Occupational Therapist to join our Community Mental Health Team. This role involves assessing, planning, and delivering care to service users, with a strong focus on individualized and holistic care to support recovery and improve quality of life.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalised care plans that address mental health and functional needs.
  • Collaborate with service users, carers, and multidisciplinary professionals to implement and evaluate individualised care.
  • Conduct clinical risk assessments and develop person-centered care plans.
  • Provide therapeutic interventions and advocate for the needs and rights of service users.
  • Supervise non-qualified staff and contribute to developing mental health knowledge within the team.
  • Work closely with community-based services, social services, and healthcare agencies to ensure comprehensive care.
  • Participate in new patient assessments and help integrate care pathways across services.
