About the Role
We are seeking an experienced Advanced Nurse Practitioner to join our Primary Care Network in North Watford. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our health promotion and wellbeing programmes.
Main Responsibilities
- Develop and implement health promotion and wellbeing programmes to improve patient outcomes.
- Manage patients with acute and chronic medical conditions, providing expert advice and care.
- Implement and evaluate individualised treatment plans for chronic disease patients.
- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide advanced nursing care to patients, including wound care and specialist clinics.
- Undertake the collection of pathological specimens and process and interpret test results.
- Provide travel medicine services and request pathology services as necessary.

Person Specification
Essential Qualifications
- Registered Nurse with the Nursing and Midwifery Council.
- Qualified Independent Nurse Prescriber on the NMC register.
- Masters degree required for qualification post December 2020.
- Minor illness qualification.
Essential Experience
- Experience of working as a practice nurse or community nurse.
- Venepuncture, ECGs, new patient medicals, chaperon procedure, diabetes, hypertension, CHD, women's health.
- Wound care/removal of sutures and staples.
