About the Role

We are looking for an experienced professional with expertise in providing assessments and interventions for individuals with diverse physical, mental, and social care needs. As a Rehabilitation Specialist, you will work closely with service users, their families, and multi-disciplinary teams to enhance independence and quality of life through tailored therapy solutions.

Key Responsibilities
  • Conduct comprehensive assessments of service users' physical and mental health needs, focusing on major and minor home adaptations.
  • Carry out assessments and recommendations under the Disabled Facilities Grant (DFG) legislation and process.
  • Provide expert advice on designing, providing, and implementing adaptations to homes, workplaces, and other environments.

This role involves ensuring compliance with Safeguarding and the Mental Capacity Act (MCA) in all assessments and care plans. You will work in alignment with the Care Act to assess and plan for care and support needs.
