About the RoleThe successful candidate will have a strong background in primary care and experience in chronic disease management. You will be responsible for assessing, planning, implementing, and evaluating treatment programs for our patients. Your duties will include:
- Developing and implementing health promotion and well-being programs
- Undertaking child vaccination programs
- Identifying and managing patients at risk of developing long-term conditions
- Providing routine nursing care to patients
- Providing guidance on medication use and side effects
- Overseeing wound care services
- Requesting and processing pathology results
- Maintaining accurate clinical records
- Ensuring read codes are used effectively
- Developing and implementing well woman and well man clinics
- Chaperoning patients
- Assisting GPs with minor surgery
- Prioritizing health issues and intervening appropriately
- Supporting the team in clinical emergencies
- Recognizing and referring patients with mental health needs
- Implementing vaccination programs
- Liaising with external services to support vulnerable patients
- Delegating clinical responsibilities
- Supporting safeguarding matters
- Delivering health promotion and managing chronic disease
- Conducting home visits and clinical appointments
- Developing and reviewing audit protocols and nursing protocols
- Supporting the nursing team and participating in shared learning
- Reviewing clinical practices and participating in local initiatives
We are looking for a qualified and experienced Practice Nurse who:
- Is registered with the NMC and has a valid PIN
- Has experience working in a primary care environment
- Has experience in chronic disease management
- Has experience working autonomously
- Has experience in infection prevention and control measures
We offer a competitive annual salary range of £18 to £20 per hour, depending on experience, skills, and qualifications. You will also have the opportunity to work in a close-knit clinical team, with full support from experienced GPs, ANPs, PNs, and HCAs. We also offer enrolment to the NHS pension scheme and flexibility in working hours.
