Job Title: Care Homes/Community Specialist Dietitian
Job Summary:
We are seeking a highly skilled and experienced Dietitian to join our team at Central London Community Health Trust. As a Care Homes/Community Specialist Dietitian, you will play a vital role in providing nutrition support to patients in community settings.
Key Responsibilities:
- Contribute to our extensive GP clinic service, working as an active member of the primary healthcare team.
- Develop your skills in the care of patients with Diabetes, Obesity, feeding difficulties, malnutrition, etc.
- Specialize in Home Enteral Nutrition, with extensive input across Harrow.
- Deliver group teaching sessions for patients, especially in diabetes.
- Train a wide range of community health professionals, such as practice nurses, community nurses, and nursing home staff.
- Hold a full, valid UK driving licence and have access to a car to use for business purposes.
Requirements:
- 4-year Degree in Dietetics or Equivalent.
- Current State Registration with the HCPC.
- Significant clinical experience following graduation, with a broad range of patients across a variety of clinical settings.
- Experience of working as part of a team.
- Previous experience of conducting audits and/or research.
- Experience of student training.
- Experience of working with people with diabetes.
- Experience of working with people from a range of ethnic groups.
Skills and Knowledge:
- Sound clinical knowledge.
- Evidence of ongoing continuing Professional Development.
- Sound Knowledge of current issues relating to Nutrition.
- Sound knowledge of Type 2 diabetes.
- Able to prioritize own workload.
- Able to work under own initiative.
- Able to achieve deadlines.
- Excellent oral and written skills with people from a wide variety of backgrounds.
- Good organizational skills.
- Training of Health Professionals.
- Good time management.
- Understanding the importance of maintaining Confidentiality.
- Organizing and delivery group sessions to patients.
Other Requirements:
- Computer literacy.
- Willingness to travel across the Borough of Harrow.
