About the Role:

We are seeking a skilled Registered Practical Nurse to join our team in a community nursing role. As a member of our team, you will have the opportunity to work with patients of all ages, providing scheduled visits and care in various settings.

Key Responsibilities:

  • Provide patient care during scheduled visits, using appropriate assessment tools and techniques to assess individualized needs.
  • Execute and continuously monitor care plans, engaging patients through a collaborative approach.
  • Maintain and update professional patient records related to patient encounters and plans.

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience in delivering community care is an asset.
  • Reliability and flexibility.
  • Willingness to travel to and from patient residences within a designated area.
  • Passion for making a difference in your community.
