About the Role:
We are seeking a skilled Registered Nurse to join our team as a Community Health Practitioner. In this role, you will provide high-quality patient care in various settings, working with patients of all ages and complexity of care needs.
Key Responsibilities:
- Conduct thorough assessments of patients' individualized needs using appropriate tools and techniques.
- Develop and implement personalized care plans, continuously monitoring and updating patient records.
- Build strong relationships with patients and their families, using a collaborative approach to deliver compassionate care.
Requirements:
- Registration with the CNO as a Registered Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a passion for making a difference in your community.
