Job Overview:

Oxford Health NHS Foundation Trust is seeking a skilled and compassionate Community Nurse to join our team of dedicated healthcare professionals. As a Community Nurse, you will provide high-quality, patient-centered care to individuals with complex health needs in their own homes.

Main Responsibilities:

  • Develop and implement holistic care plans in collaboration with patients, families, and multidisciplinary teams.
  • Provide evidence-based interventions to manage acute and chronic conditions, including wound care, medication administration, and end-of-life care.
  • Work closely with community matrons, therapists, and other healthcare professionals to coordinate care and prevent hospital admissions.
  • Conduct thorough assessments, identify patient needs, and develop individualized care plans to meet those needs.
  • Maintain accurate and up-to-date records, adhering to clinical governance requirements and Trust policies.
