About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Berkshire Healthcare Foundation Trust. As a Clinical Pharmacist, you will play a vital role in providing high-quality, specialized clinical pharmacy services to our inpatient community health wards.

Key Responsibilities

  • Provide expert clinical pharmacy services to patients, including medication management and optimization.
  • Participate in consultant-led ward rounds to support the medicines optimization agenda.
  • Develop and implement policies and procedures to ensure safe and effective use of medicines.
  • Collaborate with multidisciplinary teams to improve patient outcomes and reduce medication errors.
  • Contribute to the development and implementation of clinical pharmacy services and initiatives.

Requirements

  • Good Pharmacy Honours degree (BPharm or MPharm)
  • Postgraduate Certificate in clinical pharmacy
  • Postgraduate diploma or Master's degree in clinical pharmacy (Band 8a)
  • Registration with the General Pharmaceutical Council (GPhC)
  • Member of the Royal Pharmaceutical Society
  • Independent prescriber
