Service Care Solutions is seeking a dedicated individual to join our team as a Leaving Care Personal Advisor for Manchester City Council.
**Key Responsibilities:**
* Provide high-quality support to young people aged 16 and over who have entitlement under the Children Leaving Care Act (2000)
* Deliver effective support services, managing existing and future risks through Pathway Plan assessments
* Create personalized plans that consider the individual needs and strengths of young people
* Facilitate engagement with support services provided by statutory and voluntary agencies
* Offer expert advice and representation to promote the rights and needs of young people

* Experience working with care-experienced young people
* Knowledge of the Children Leaving Care Act (2000)
* Excellent communication and interpersonal skills
