**Job overview**:
As a Specialist Palliative Care Nurse, you will provide evidence-based specialist palliative care and advice to patient, families and staff to ensure the delivery of high quality, person centred palliative care over 7 days.

The team links closely with other teams and departments across the trust and more widely across organisational boundaries.

You will also support the delivery of palliative and end of life care education.

In return you will join a supportive team and be supported to develop within the role.

**Main duties of the job**:

- Manage a caseload of patients with specialist palliative care needs.Use specialist knowledge to provide advice and support around complex palliative care including symptom management and psycho-social support.

Establish and maintain highly effective communication with patient’s, families and professionals including dealing with sensitive and complex issues.

Support the provision of education delivery across professional disciplines.

Participate in relevant audit and research.

**Essential criteria**:

- Current NMC registration first level nurse
- Post registration qualification in palliative care or relevant courses / study days
- To have undertaken relevant communication skills training (Sage and Thyme or equivalent)
- Registered mentor
- Demonstrable or significant experience in palliative care or evidence of post qualification experience
- Effective communication skills, both verbal and written
- Be an effective team member who is able to relate to colleagues at all levels of the Trust with tact and diplomacy
- Demonstrate an understanding of and involvement in current palliative care issues
- Effective management / leadership skills
- Effective time management skills and the ability to prioritise own workload
- Ability to work autonomously as well as part of a multidisciplinary team
- Demonstrate an understanding of managing change in clinical practice
- Basic computer literacy
- (Patient and customer focus, continuous improvement, accountability, respect)

**Desirable criteria**:

- To have undertaken 2 day Enhanced Communication Skills training, or equivalent
- Willing to undertake independent & nonmedical prescribing module
