Job Title: Community Care Services Coordinator

Rutland County Council is seeking a dedicated and compassionate individual to join our team as a Community Care Services Coordinator. This role will provide operational support to the setup of Rutland's Shared Lives project, working closely with Project Leads and Project Managers to develop proposed structures and ensure the successful implementation of the scheme.

Key Responsibilities:
  • Support the development of proposed structures for the Shared Lives project
  • Provide operational support to the setup of the project, working closely with Project Leads and Project Managers
  • Collaborate with local authority social care processes to ensure effective delivery of the scheme
  • Work with approved carers to match individuals with care and support needs with suitable placements
Requirements:
  • Experience working within a regulated care provider or commissioning authority
  • Knowledge of and commitment to person-centered approaches to care and support
  • Knowledge of legislation, policy, and practice relevant to services for adults with care and support needs
  • Qualification in Health or Social Care to NVQ Level 3 or equivalent
