About the Role

We are seeking a highly motivated and experienced Support Worker to join our team at Cartrefi Cymru Co-operative. As a Support Worker, you will play a vital role in supporting individuals with learning disabilities, mental health needs, older people, people with Autism, and physical disabilities across Wales.

Key Responsibilities
  • Support individuals to live fulfilled and independent lives within their homes and the wider community.
  • Provide high-quality support and care to individuals, working closely with their families and Social Services.
  • Develop and maintain relationships with individuals, their families, and community partners to ensure effective support and care.
  • Contribute to the development and implementation of individual support plans, ensuring that they are tailored to meet the unique needs of each individual.
  • Work collaboratively with the Area Manager to deliver on Cartrefi Cymru's promise to build relationships and partnerships in the community through activities in community hubs across Carmarthenshire.
Requirements
  • A commitment to enabling people to live fulfilled and independent lives.
  • A full valid driving license and a car, which you are able to use for work purposes as driving is essential.
  • Level 3 / 5 qualification in Health & Social Care.
  • Good organisational skills and ability to prioritise workload and meet deadlines.
  • Good communication and networking skills, with the ability to deal with people at all levels.
  • Able to think creatively and build on opportunities.
  • Computer literate and have a thorough knowledge and experience of working with Microsoft Office packages, including Word, Excel, and Outlook.
  • Fully flexible over 7 days, with evening and weekend work required.
