Early Intervention Psychosis Care Coordinator Role

This vacancy is suitable for qualified social workers, occupational therapists, or mental health nurses. As a Care Coordinator for the Early Intervention in Psychosis (EIP) Service, you will work with individuals experiencing psychosis for the first time, delivering personalized and meaningful care.
Main Responsibilities
As a Band 6 Care Coordinator, you will work together with colleagues to provide the PATH service, covering a designated locality and traveling less to work out in the community. You will also train as a Family Intervention Therapist.
About the Role
You will be passionate about delivering recovery-based and collaborative approaches to care planning, working flexibly and creatively with people to achieve their individual goals. Our 'Caseload Mini Team' workforce model is unique and highly supportive of collaborative working.

Key Skills and Qualifications
To be successful in this role, you will need to have education to degree level or equivalent, registration with NMC/HCPC/SWE, and solid post-registration experience, including Band 5 and 6. You will also need excellent communication skills and experience of using electronic recording systems.
