We are seeking a highly skilled and compassionate Registered Physical Health Nurse or General Nurse or Practice Nurse to join our team at Elysium Healthcare as a Practice Nurse. As a Practice Nurse, you will play a vital role in promoting health and wellbeing for individuals with complex needs.
Key Responsibilities- Lead and Coordinate Physical Health Needs: You will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team.
- Monitor and Assess: You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented.
- Develop and Implement Interventions: As a Practice Nurse, you will develop and support physical health interventions used in the clinical area using knowledge of current best evidence-based practice.
- Collaborate with Primary Health: You will work collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
- Monitor and Maintain Dashboards: You will be responsible for monitoring and maintaining Physical Health Dashboards.
- Nursing Qualification and Registration: You will have a relevant nursing qualification and registration.
- Phlebotomy Skills: You will have phlebotomy skills.
- Strong Team-Working Skills: You will have strong team-working skills.
- Self-Motivation and Flexibility: You will have a high level of self-motivation and a flexible approach.
- Positive Attitude and Compassion: You will have a positive attitude and be naturally caring and compassionate.
- Commitment to High-Quality Care: You will have a commitment to high-quality care and to values and evidence-based practice.
- Understanding of Physical Health Care Needs: You will understand the physical health care needs for individuals who are living with a acquired brain injury.
- Annual Salary: An annual salary of £34,590.
- Working Hours: You will work Monday to Friday 9am - 5pm.
- Annual Leave: The equivalent of 33 days annual leave (inc Bank Holidays) – plus your birthday off.
