About Our Team
Pennine Care NHS Foundation Trust is a leading provider of mental health and learning disability services across Greater Manchester. Our team is dedicated to delivering high-quality care and support to individuals with complex and severe mental health needs.
We are seeking a skilled and experienced Adult Mental Health Practitioner to join our team, working closely with patients, carers, and other professionals to provide comprehensive assessments, develop personalized care plans, and deliver interventions that promote recovery and reduce stigma.
The successful candidate will possess excellent assessment and intervention skills, as well as the ability to communicate effectively with individuals from diverse backgrounds. You will be a registered mental health nurse, occupational therapist, or social worker with a strong background in mental health practice, preferably in a community or primary care setting.
Key Responsibilities
- Conduct comprehensive assessments of patients' needs, identifying strengths and areas for improvement, and develop personalized care plans that address their complex mental health and physical health needs.
- Work collaboratively with colleagues from psychiatry, psychology, and other disciplines to deliver holistic care and support, ensuring seamless transitions between services.
- Provide education and training to peers and assistants, sharing knowledge and expertise to enhance service delivery and improve patient outcomes.
Requirements
- RMN/Social Worker/OT qualification and registration with relevant professional body.
- Demonstrable post-qualification experience in mental health, preferably in a community or primary care setting.
- Excellent assessment and intervention skills, with the ability to communicate effectively with individuals from diverse backgrounds.
- Ability to work effectively as part of a multidisciplinary team, with a commitment to delivering person-centered care.
What We Offer
We offer a competitive salary of £38,890 - £44,884 per annum, depending on experience. In addition, we provide a range of benefits, including access to training and development opportunities, a generous pension scheme, and flexible working arrangements.
