Sheffield Community Care Coordinator

Are you passionate about improving quality of life for older people in Sheffield? Do you have a solution-focused approach and excellent time management skills? If so, this role as a Sheffield Community Care Coordinator could be perfect for you.
As a Sheffield Community Care Coordinator, you will support individuals who have had recent hospital admissions to help them continue living independently at home. You will develop strong relationships with older people, GP networks, hospitals, partner and referral organisations to facilitate access to services, resources and facilities.
You will receive a bespoke induction and training to equip you with the knowledge and skills to complete holistic assessments and provide solutions to identified issues. You will also benefit from shared learning sessions and external reflective practice sessions.
The salary for this role is approximately £24,767 per annum, plus pension and 30 days annual leave plus public holidays. You will work 36.25 hours per week, flexibly between Monday to Friday, 8am-6pm.
